NSAP计划中多半资金（每年5亿美元中的3亿）用于购买抗艾滋病的药品，巴西政府因此想尽各种办法降低药品采购费用。政府建立了国家实验室和国有药品制造厂以促使医药生产本地化，现在巴西自己已经能够生产“鸡尾酒”疗法所用的15种药品中的 7 种，这7种药品在巴西都没有申请专利，因此在药品采购费中本地生产的药品所占份额不大，1999 年只占 19%而其他 81%的药品采购费都用在了购买跨国公司生产的专利药品上。为了进一步降低药品采购费用，巴西政府威胁对一家美国制药公司 Merck 和一家瑞士制药公司 Roche 生产的专利药品（分别是 Stocrin 和 Viracept）颁发强制许可，这两家公司被迫大幅降低了售价。
